Coxsackie-Athens High School

School districts in New York State are responsible for issuing employment certificates and permits to qualified applicants. This is not a function of the State Education Department. The forms necessary for applying for the permit can be found below.

Minors may apply for certificates where they live, where they go to school, or where they are to be employed.

Physical Fitness Certification : This is know as the 'AT16' and must be certified by either the applicant's primary care doctor or our school nurse.

Application For Employment Certificate : This is known as the 'AT17' and an applicant's parent must only fill put Part I.

Once these two forms are completed to the specifications listed above, they are to be brought to Mrs. Weinstein in the High School Counseling Office for verification or scanned & emailed to her at [email protected]. 

At that time, she can issue the working permit to the applicant and she will call you to confirm when it is ready to be picked up.  The applicant must be present to receive the permit because he or she must sign in a school official's presence.

Hard copies of each form are available at the High School Main Entrance if you do not have the means to print them off.
